Completing your application

Once you’ve been evaluated by a qualified physician, you will be entered into the Medical Marijuana Use Registry (MMUR) and will receive an email to complete your ID card application. There is a $75 fee to complete the application.

How do I get a medical cannabis card?

If you have a qualifying condition, obtaining a medical cannabis card is a fairly simple process. You’ll need your medical records and a signed physician’s statement from a doctor who has a Florida medical marijuana license and is registered with the Medical Marijuana Use Registry.

Once you have met with your doctor and he or she has deemed you eligible to use medical cannabis, the doctor will enter your information into the Medical Marijuana Use Registry.

Once you are entered in the Registry, you can submit a completed application to the Office of Medical Marijuana Use, or you can have your doctor (or a legal representative) submit it for you.

What are the qualifying conditions in Florida?

“Qualifying conditions” are medical conditions that the state of Florida has determined can be treated successfully with cannabis medicine. In order to obtain a medical cannabis card, you’ll need to demonstrate that you suffer from one or more of these conditions. The current qualifying conditions for medical cannabis in Florida include:

  • Amyotrophic Lateral Sclerosis (Lou Gehrig’s Disease)
  • Cancer
  • Crohn’s Disease
  • Chronic Muscle Spasms
  • Epilepsy
  • Glaucoma
  • HIV/AIDS
  • Multiple Sclerosis (MS)
  • Parkinson’s Disease
  • Post-traumatic Stress Disorder (PTSD)
  • Seizures
  • Pain caused by or that originates from a qualifying condition
  • A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification
  • Any other condition of the same severity and/or symptoms, when determined by a doctor’s opinion that the medical use of cannabis would surpass any potential health risks
  • “Other debilitating medical conditions”

Additionally, Amendment 2 allows physicians to certify patients for a medical cannabis recommendation after diagnosing them with “other medical conditions of the same kind or class or comparable to those enumerated.” These may include:

  • Anxiety
  • Anorexia
  • Arthritis
  • Back Pain
  • Cachexia (Wasting Syndrome)
  • Cyclical Vomiting Syndrome
  • Diabetes
  • Hepatitis C
  • Irritable Bowel Syndrome (with chronic abdominal pain)
  • Lyme Disease
  • Migraine
  • Muscle Spasms
  • Muscular Dystrophy
  • Severe & Chronic Pain
  • Severe Nausea
  • Sickle Cell Anemia
  • Spasticity
  • Any Terminal Condition
  • Other Condition of Similar Kind Or Class
